To ensure prompt processing, please include the following information when placing orders via
Mail, Email and Fax.
- Account Number
- Billing and Shipping addresses (if different)
- Purchase Order Number (if applicable)

Minimum Order Requirements
PMT Medical does not have a minimum order requirement. We understand how hard it is to meet
these types of volume. No order is too small or too large for PMT Medical.
Same-Day Shipping
Call by 1:00 p.m. (EST) and your order will be shipped via U.P.S. Ground that same day. Next Day Air,
2nd Day Air and 3-Day Select are also available upon request.
Freight Policy
Shipping and handling charges include UPS charges and a small handling charge. Shipments outside
the continental United States will be billed accordingly.
Drop Shipments
PMT Medical will ship directly to YOUR customer, on your behalf. Your customer will receive your
packing slip with their order showing your company name and a list of products shipped. There is no
pricing and no reference to PMT Medical on the packing list.

For your convenience, PMT Medical offers the various forms of payments listed below:
- Open Account These accounts are subject to PMT Medical's open account
terms and satisfactory credit references.Terms Net 30 Days.
Credit Application
- Credit Card Purchases We accept VISA, MasterCard, and Discover. There is not a
service fee if paid at the time of order.
- C.O.D. Purchases PMT Medical will ship your order C.O.D.
(except for Drop Shipments) and accepts cash, a certified
bank check or money order.PMT Medical will accept C.O.D.
company check with prior credit approval.
- TeleChek Save C.O.D. charges, call for details.
- Wire Transfers Call for details.
Contract Pricing
Contract pricing is available for volume commitments by accounts. Ask a member of our friendly Customer
Care Team for details.
Customer Satisfaction
We know healthcare and understand the unique needs of your customers. Our staff of professionals will
assist you in answering questions concerning new or current products.
Return Policy
Please call for a written authorization number, valid for 30 days. Include a copy of your original invoice
with your return.
- Full credit is issued for stocked items ordered in error when returned in resaleable condition
within 30 days.
- Fifteen percent (15%) restocking charge for returns of overstocked items when returned in resaleable
condition after 30 days.
- For any returns of special order items or returns over 60 days, please call for details.
- Notification of shortages and damages must be made within 48 hours of receipt.
